It's not an automatic update, just an update notification. When you download the update, you are just downloading the installer exactly the same as if you went to www.opera.com/download and downloaded it. It runs just like a regular installer would. You have to close the program before trying to update it (once the installer has started up), otherwise the OS refuses to allow the installer to overwrite the files with their new version (Firefox gets around this by forcing you to close it in order to run the update). That is the usual cause of update problems, so I assume that is what is happening for you.
